We do know that provider consolidation increases prices but the evidence about insurance consolidation is sketchier. As you point out in the blog, lower fee schedules might be acceptable because few enough people would be covered to matter much or Exchange Medicare might face enough provider resistance to force prices up. Either way, Exchange Medicare would not become a re-pricing yardstick.
